                                           For our Patients who don’t have dental insurance.

                                     For only $500.00 per year, you receive the following benefits:

            Two cleanings per year                                                       Two dental exams per year

            One set of dental bitewing x-rays                                       10% off any dental services

       To receive discounts on treatment, payment is due when services are rendered. Services must be redeemed within 1 year (365 days) from date of payment of plan & cannot be carried forward.
