Patient Billing

For your convenience we accept Visa, MasterCard and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.  Co-pays are due at the time of service – We will bill your insurance as a courtesy to your but it is your responsibility to be sure you are covered for your visits. 

PAYMENT OR PATIENT PORTIONS OF INSURANCE IS EXPECTED AT THE TIME OF SERVICE.  EFFECTIVE MARCH 1, 2020, THERE WILL BE A 18% APR INTEREST CHARGE PER MONTH ON ANY PAST DUE ACCOUNTS.  PLEASE MAKE ARRANGEMENTS TO TAKE CARE OF ANY OUTSTANDING BALANCES AT THIS TIME. 

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan but this must be done prior to the actual procedure.

       To receive discounts on treatment, payment is due when services are rendered. Services must be redeemed within 1 year (365 days) from date of payment of plan & cannot be carried forward.
